Early Intervention Service Coordination

Children ages 0-3 found eligible for Early Intevention through St. Lawrence County Public Health can select Cerebral Palsy Association of the North Country to provide service coordination. The service coordinator assists the family to access early childhood programs to promote health and mental health, educational, social, and other community based services needed by and provided to children and their families. As the child ages, the service coordinator explores future transition into appropriate programs for your child. Eligibility requirements for children enrolled in this program are as follows:

  • Must be under 3 years of age
  • Have a confirmed disability or established developmental delay in one or more of the following areas of development:
    • physical
    • cognitive
    • communication
    • social-emotional
    • adaptive
